One specific wrist injury that can occur after falling on an outstretched hand is called scapho-lunate dissociation. A person who experiences pain in the back of the wrist or the thumb side of the wrist (following a fall) should be examined for scapho-lunate dissociation. Our hands (right beside the area of the wrist) have many different bones in it. Scapholunate dissociation, also known as rotary subluxation of the scaphoid, refers to an abnormal orientation of the scaphoid relative to the lunate, and implies severe injury to the scapholunate interosseous ligament and other stabilizing ligaments. Carpal dissociation implies carpal instability, which has important clinical implications; thus it is essential to identify this finding on imaging.

The most prominent signs of SLAC wrist are the injury in scapholunate ligament or scaphoid which spreads out radially in the wrist. Osteoarthritis of wrist is usually caused as a result of SLAC wrist. SNAC of the left wrist caused by a displaced fracture of the distal pole of the scaphoid is reported with ICD-10-CM code S62.012K Displaced fracture of distal pole of navicular (scaphoid) bone of left wrist, subsequent encounter for fracture with nonunion. Although "scapholunate dissociation" is a descriptive term, it is used synonymously with its primary pathophysiologic correlate, scapholunate interosseous ligament (SLIL) injury. Although injury to additional ligaments is present in scapholunate dissociation, the SLIL is the major target of surgical reconstruction.

Multiple surgical procedures have been designed to reduce the scapholunate interval, restore normal wrist Mayfield classification of carpal instability, also known as perilunate instability classification (carpal dislocations), describes carpal ligament injuries.. Instability has been divided into four stages 1-2:. stage I: scapholunate dissociation (rotatory subluxation of the scaphoid) disruption of the scapholunate ligament with resultant Terry Thomas sign tion showed severe degenerative joint disease of the left wrist with scapholunate dissociation, scapholunate ligament tear, and avascular necrosis of the lunate (Figure 1). The patient was subsequently treated with corticosteroid injection of the wrist which only provided relief for less than a … diastasis of the scapholunate complex occurs with complete SLIL tears and capsule disruption. Associated injuries. DISI (dorsal intercalated segmental instability) scapholunate dissociation causes the scaphoid to flex palmar and the lunate to dorsiflex; if left untreated the DISI deformity can progress into a SLAC wrist the left wrist.
